Insurance Claims 

  • All insurance claim requests must be made through the form below. Please take the time to fill it in accurately.

  • Please ensure you have read through and understand the terms and conditions relating to insurance claims processing before proceeding.
     

  • If you are claiming for more than two conditions at any one time, please complete an additional claim form.  Please note that additional admin fees may apply.

  • If your pet is on our health care plan 3 x standard claim admin fees are included per year as part of your pet’s health plan package.

Service being requested:
Standard claim: I have paid all outstanding bills. Veterinary input is required to submit my claim. The claim settlement will be paid to the policy holder (£6.50 admin fee applies).
Direct claim (For unpaid invoices over £500 only): A vet has agreed to submit a direct claim on my behalf. The claim settlement will be paid to The Grove Vets. I agree to my debit/credit card to be kept on file until the claim is settled. (£25 admin fee applies).
I am submitting my own claim. I have paid all outstanding bills and I can confirm that no veterinary input is required for my claim to be processed. I have confirmed that my insurance provider allows this. I would like to request a copy of my invoices and pet's clinical history only. (No admin fees apply).
