Please note that if you are claiming for more than 2 conditions within the same claim period, you will need to submit an additional request form (Please note this will be subject to an additonal admin fee if applicable).

Service being requested:

I have paid all outstanding bills and I am processing the claim myself (please check that your insurance company allows this) and would like to request a full clinical history and invoices between the following dates. (Please note if you select this option we will not be in a position to separate invoices by condition). (£0)

I have paid all outstanding bills and require The Grove Vets to process a claim on my behalf, with settlement funds sent to myself. (£6.50)

A vet has agreed to process a direct claim, with settlement funds sent directly to The Grove Vets (£25.00). I am aware I will need to leave details for a valid credit/debit card on file until final settlement has been made.
